Camper Physical

To ensure your child is healthy enough for camp activities, we require that your child has had a physical completed within the last 12 months.  A sports/school physical or a physical performed by your physician will suffice. If a copy of their physical is not available, you may download the Health Form below to be completed by your child’s physician.

The form should be submitted via the Upload function in your online registration portal.
